On the Add a new question page, you enter:

1. The question code (any value that is meaningful to you -- like Age, Date, whatever you want)
2. The question text
3. Select the question type
4. Click Save at the top right of the page

On the Question Summary page, click the button "Edit answer options" in the top row. From there you can enter your answers.

For Multiple choice and Array, same idea, from the Question Summary page, click the "Edit subquestions" button to add your options.
