Once you've created at least one question group, you'll be able to add questions to your survey. If you've just created a Question Group, (and if you have more than one question group, once you select a question group) you'll see the Question toolbar:

Click on the Add New Question icon to add a question.

An icon bar will be displayed giving you full control (including preview ability and logic branching):

Hover over each icon for a tool tip. The red/yellow/green branching icon is used to build branching logic into your questions -- this lets you set rules to determine if a question shoud be displayed, based on responses to previous question(s).

A question is comprised of:

• A response code
• The question text (for example: What is your favourite colour?)
• Help text if you wish to provide any.
• Question Type (see full descriptions on the LimeSurvey wiki) -- the system will provide you with preview images of each question type to help you select the appropriate type.
• Mandatory
• Validation

You also have the option of viewing Advanced Settings for each question. This gives you extensive control over each question and its behaviour.
